REGISTRATION FORMS ARE AVAILABLE AT THE TNR OF WARREN CLINIC

-Low income pet $40
-Outside cat in carrier $30
-Trapped cat $20
Rabies shots included on all pets.

If you have more than one cat you will need a form for each cat.

 

+ You must have your cat(s) at the TNR of Warren Spay & Neuter Clinic 2428 Elm Road, NE, Warren, Ohio

+Must drop off your cat(s) from 8am-9am

+ No food or water after 10 PM

 

+ Must be in a clean, secure carrier (no soft sided carriers) and either newspaper or clean bedding in the carrier.

 

+ Fill out all blanks on the form to the best of your knowledge.

 

+ Put a piece of paper on the carrier and on it write your NAME, CONTACT PHONE, CAT'S DESCRIPTION, FEMALE OR MALE, & FERAL if it is wild.

 

+ Place a second piece of paper on the carrier with the CAT'S NAME IN LARGE LETTERS.

+ ALL outside cats will be ear-tipped. No exceptions.

 

Please come to the office to prepay and preregister.

NOTE Criteria for Eligibility for low-income pricing:

  1. Section 8 housing voucher

  2. Schedule letter from Ohio Works First

  3. Ohio Medicaid card 

  4. Letter from Veteran Benefits Administration acknowledging service-related disability

  5. Ohio Electronic Benefit Transfer (EBT) Card, also called the Direction Card, for supplemental nutrition assistance program, SNAP (previously referred to as “food stamps”)

  6. WIC card (Women Infants and Children nutrition program)

  7. Benefit letter documenting Supplemental Security Income

  8. Benefit letter documenting assistance from Social Security Disability

  9. Certificate of adoption showing that the animal was adopted from a licensed animal shelter or pound.

  10. Certificate of adoption showing that the animal was adopted from a nonprofit corporation.

  11. Tax return showing owner’s family income does not exceed 150% of the federal poverty guideline.
